Breast Augmentation (Implants)
What it does: Adds volume to breasts that are naturally small, have lost volume after pregnancy and breastfeeding, or have asymmetry that creates a visible size difference.
Implant types: Silicone gel implants are the most commonly used in India and globally. They feel natural, have a low complication rate, and come in a range of sizes and profiles. Round implants are the standard for most patients. Anatomical (teardrop) implants are used selectively when a more natural slope is needed.
Placement: Submuscular (behind the chest muscle) is Dr. Ritesh’s preferred placement for most patients. It provides more natural coverage over the implant, lower capsular contracture risk, and better long-term aesthetics, particularly in thinner patients with less natural breast tissue to camouflage the implant edges. Subglandular (in front of the muscle) may also be appropriate in specific cases.
Incision options: Inframammary fold (in the crease beneath the breast) is the most common and provides the best access for precise placement. Periareolar (around the lower edge of the areola) is an alternative that hides the scar within the colour transition zone. Dr. Ritesh selects the incision based on implant size, breast anatomy, and patient preference.
Sizing: This is where surgical judgment matters most. Dr. Ritesh uses various sizes breast pictures during the consultation to help patients visualise the result rather than choosing from a catalogue or an app. The goal is a size that looks natural & proportional to your frame and existing anatomy and not one that looks impressive in isolation. Over-sizing is the most common regret in breast augmentation, and it’s entirely preventable with a conservative, anatomy-first approach.
Recovery: One to two weeks off work. Sports bra worn for 4 to 6 weeks. No heavy upper body exercise for 6 weeks. Implants settle into their final position over 3 to 6 months (“drop and fluff”).

Breast Reduction
What it does: Removes excess breast tissue, fat, and skin to create a smaller, lighter, more proportional breast. Repositions the nipple to a natural height. Reshapes the remaining tissue into a lifted contour.
Who it’s for: Women with disproportionately large breasts causing chronic neck pain, back pain, shoulder grooving from bra straps, skin rashes beneath the breast fold, difficulty exercising, self-consciousness, and postural problems. Breast reduction is one of the most functionally impactful procedures in plastic surgery. Patients consistently describe it as life-changing, not in the way cosmetic procedures usually are, but in the sense of being able to run, sleep comfortably, and wear clothing without pain for the first time in years.
Insurance: Breast reduction is usually not covered by health insurance providers in India.
Scars: Breast reduction leaves scars around the areola, vertically below it, and sometimes in the breast fold (the anchor pattern). These are the trade-off for the procedure. Scars mature and fade over 12 to 18 months with proper scar management. For most patients, the relief from symptoms far outweighs the scar trade-off.
Recovery: Similar to augmentation. One to two weeks off work. Sports bra for 6 weeks. The weight lifted from the chest and shoulders provides noticeable postural relief from the first week.

Breast Lift (Mastopexy)
What it does: Repositions breasts that have sagged (ptosis) without adding or removing significant volume. Lifts the nipple back to a natural position above the breast fold. Reshapes the breast tissue for a firmer, more youthful contour.
Who it’s for: Women whose breast volume is adequate but the position has dropped. This typically happens after pregnancy and breastfeeding, after significant weight loss, or simply with age. The defining sign: the nipple sits at or below the level of the breast fold. The breast looks deflated or drooping rather than small.
Scar patterns: The degree of ptosis determines the scar pattern. Mild ptosis can be addressed with a periareolar scar (around the areola only). Moderate ptosis requires a lollipop scar (around the areola plus a vertical line below it). Significant ptosis requires an anchor scar (periareolar plus vertical plus horizontal in the breast fold). Dr. Ritesh uses the minimal scar pattern that achieves the necessary lift, because more scar does not mean better lift. It means more scar.
Important distinction: A breast lift does not add volume. If the breast has both sagged and deflated (which is common after breastfeeding), a lift alone will reposition a flat breast higher on the chest but won’t restore fullness. That scenario needs lift plus augmentation (see below).
Recovery: One to two weeks off work. Sports bra for 6 weeks. Final shape settles over 3 to 6 months.

Breast Lift + Augmentation Combined
What it does: Combines the repositioning of a breast lift with the volume restoration of implants or fat grafting in a single procedure. Addresses both sagging and deflation simultaneously.
Who it’s for: The most common scenario is the post-pregnancy patient whose breasts have both dropped and lost volume. She looks at her chest and sees two problems: the position is lower than it should be, and the fullness she had before pregnancy is gone. Augmentation alone would add volume to a drooping breast, making it heavier and potentially sagging more. A lift alone would reposition a flat breast higher but leave it deflated. The combination solves both.

Recovery: Slightly longer than either procedure alone, because both the implant pocket and the tissue reshaping need to heal simultaneously. Two weeks off work. Sports bra for 6 to 8 weeks. Final shape at 4 to 6 months.

Fat Transfer Breast Augmentation
What it does: Fat is harvested from the patient’s own body (typically abdomen, flanks, or thighs) via liposuction, processed to isolate viable fat cells, and then injected into the breast to add volume. No implants. No foreign material. The augmentation is built entirely from your own tissue.
Who it’s for: Patients who want a modest volume increase (typically one cup size) without implants. Patients who want a natural feel (fat integrates as living tissue and behaves like normal breast tissue). Patients who are already planning liposuction and have a donor site available, making the fat harvest a dual-purpose step.
Limitations: One cup size maximum in most cases. Fat survival is variable: 30 to 40 percent of the transferred fat is typically reabsorbed by the body, which means Dr. Ritesh intentionally over-injects and plans for the reabsorption. A second fat grafting session at 3 to 6 months may be needed to achieve the desired final volume. Fat transfer also cannot lift a sagging breast. If your breast has ptosis, you need a lift (with or without fat transfer for modest additional volume).

Recovery: Similar to liposuction recovery (the donor site) plus mild breast swelling for 2 to 3 weeks. Less intense than implant-based augmentation because there’s no implant pocket creation or muscle elevation.

Breast Asymmetry Correction
Almost all breasts have some degree of natural asymmetry. It becomes a surgical concern when the difference is visible enough to affect clothing choices, body confidence, or physical comfort.
Correction depends on the type of asymmetry: one breast may be significantly larger (requiring reduction of the larger side, augmentation of the smaller side, or both), one breast may sit lower (requiring a lift on one side), or both breasts may be different shapes entirely (requiring a customised combination). Who Should Wait
Still planning pregnancy or breastfeeding. Pregnancy changes breast volume, shape, and tissue composition. Breastfeeding changes it further. If you plan to have more children, scheduling breast surgery afterward ensures the result lasts. Augmentation before pregnancy is less of an issue (implants are not affected by pregnancy), but a lift or reduction performed before pregnancy may need revision afterward.
Weight not stable. Breast size fluctuates with weight. If you’re in the process of losing or gaining significant weight, waiting until you’ve maintained a stable weight for 6 months produces a more predictable surgical result.
Under 18. Breast development continues through the late teens. Cosmetic breast surgery should wait until development is complete, typically by age 18. Breast reduction for significant functional symptoms may be considered earlier on a case-by-case basis.

Breast Surgery and Breastfeeding
This question comes up in every breast surgery consultation, so here’s the clear answer:
Augmentation (submuscular /subglandular or Dual plane): Generally does not affect breastfeeding. The implant sits behind the muscle, away from the breast tissue and milk ducts.
Reduction: May affect breastfeeding depending on the technique used. If the nipple is detached and reattached as a free graft (rare in modern techniques), breastfeeding is unlikely. If the nipple remains attached on a pedicle (the standard approach), breastfeeding is usually preserved, though some reduction in supply is possible.
Lift: Generally preserves breastfeeding ability. The nipple remains attached to its blood and nerve supply during repositioning.
Fat transfer: Does not affect breastfeeding. Fat is injected into the subcutaneous tissue, not the glandular tissue.
If future breastfeeding is a concern, discuss it with Dr. Ritesh during the consultation. The technique can be selected with breastfeeding preservation as a priority.
Frequently Asked Questions
How long do breast implants last?
Modern silicone implants do not have a fixed expiration date. They are designed to last 10 to 20 years or longer. They don’t need to be replaced at a set interval unless there’s a specific issue (capsular contracture, rupture, or the patient wants a size change). Dr. Ritesh recommends regular follow-up imaging (ultrasound) starting at 5 years to monitor implant integrity.
Will breast augmentation look natural?
When sized proportionally to your frame and placed submuscularly, modern silicone implants produce a natural look and feel. The “obviously augmented” look comes from implants that are too large for the patient’s chest width or placed in front of the muscle on a thin patient. Dr. Ritesh’s sizing approach prioritises proportion over volume.
